Anyone
of sound mind who is eighteen or over may make an anatomical gift by
Will or by
signing in the presence of two witnesses a document evidencing the
intent to
make a gift. This document can be a driver's license or a donor card
available
from doctors as well as organizations.
To
be valid upon death, a donor card need not be delivered to anyone
during the
donor's lifetime. If you change your mind about making an anatomical
gift, and
your donor card or other document of gift has not been delivered to a
specified
donor, just tear it up. If your gift has been made by Will, you must
sign an
amendment to the Will to revoke the gift.
